Face looking flatter, hollow, or tired? Here's why it happens
Faces don't only wrinkle, they deflate. Fat pads shrink, bone changes shape, and collagen declines, which can leave cheeks flat, temples hollow, and folds deeper. We help you understand what changed and what, if anything, to treat first.
What might be going on
Facial aging is more than skin. Research describes shrinking facial fat compartments, gradual changes in the underlying bone, and thinner skin, which together can look like laxity or deeper folds.
- Shrinking fat pads in the cheeks, temples, and around the eyes
- Bone changes that reduce support for the soft tissue above
- Collagen decline, sun damage, and smoking
- Significant or rapid weight loss, which can reduce facial fullness
How we evaluate it
Dr. Chander examines your face at rest and in motion, from the front and the side, to see where support has been lost. We discuss what bothers you and review your history and medications. Sometimes the best first step is a smaller area or better skin quality. You'll see the plan, the products, and what to expect before anything is done.
Treatments we offer
The right option depends on where and how much volume has been lost:
- Dermal Fillers — Juvederm and Restylane hyaluronic-acid gels that add volume to cheeks, jawline, chin, and lips, and can be dissolved if needed
- Sculptra — a collagen-stimulating injectable that may build volume gradually over weeks, usually in a series of sessions
- SkinPen Microneedling — may improve skin texture and firmness, and can be paired with Sculptra
When to see your doctor first
After filler, call 911 for sudden vision changes, severe pain, or skin that turns white, gray, or blue. The FDA warns that filler entering a blood vessel can cause tissue death, vision loss, or stroke. Face drooping, trouble speaking, or weakness on one side also needs 911.
See your doctor first for unexplained weight loss, sudden facial swelling or asymmetry, or a lump or spot that is growing or changing. Tell us about prior facial surgery, other fillers, autoimmune conditions, and any infection or cold sore near the area.
Wondering what has changed in your face? Book a Complimentary Consultation. Dr. Chander will walk through the options and tell you plainly when less is more.

Will I look overfilled?
That is what we plan against. We build gradually, favor definition over volume, and let you decide how far to go.
What is the difference between filler and Sculptra?
Filler adds volume right away, while Sculptra prompts your skin to build collagen over several weeks. The right choice depends on the area and your goals.
Can filler be reversed?
Hyaluronic-acid fillers can be dissolved with an enzyme if needed. Sculptra can't be reversed that way, which is why we start conservatively.
Let's figure out what's behind it.
